New Website for Colorado Practices to Find Transformation Opportunities

Launched by Practice Innovation Program Colorado, a new online catalog and interactive self-assessment tool shows what’s available to support and advance primary care in the transformation process. Particularly helpful to primary care practices, the site includes resources for programs and partners available to assist with practice goals.

Colorado practices are invited to use this guide to create individualized learning curriculum that advances the competencies that are needed for high-performing practices. In Colorado, high-performing practices deliver measureable value to consumers, payers, and providers alike. The offerings presented in this catalog are categorized according to the content, designed in such a way that a practice, whenever it is on its trajectory, toward high-performing care, can choose a meaningful next step.

The website was developed as a collaborative effort, led by Practice Innovation Program Colorado. As the convener of the Colorado Health Extension System, Practice Innovation Program Colorado mobilizes quality improvement expertise from organizations across the state to engage practices and communities in achieving better population health, lower costs, and improved experience of care for patients, families and healthcare teams.
