Practices  in SIM & TCPi: 

A significant benefit to participating in initiatives like TCPi is the opportunity to connect with clinicians and staff from across the state who are working towards similar goals. It allows us to bring together teams to network and share their experiences with peers. Practices in Colorado have been participating in practice redesign efforts such as patient-centered medical home, improving clinical outcomes, and preparing for alternative payment models for over a decade. These practices have been coming together on a semi-annual basis to learn and share best practices with one another in the evidence-base model of learning collaboratives. The Collaborative Learning Sessions (CLS) provide a venue for your team to not only disseminate your ideas, but to gain new perspectives and network with peers. Each CLS has initiative specific breakouts with speakers who are experts in their fields. These sessions offer training on related clinical content and strategic planning to help your team improve momentum in your practice transformation goals. CME is available.

Keynote Speaker:

Paul Grundy, MD, MPH, FACOEM, FACPM

Global Director Healthcare Transformation, HealthTeamWorks

Dr. Grundy known as the "godfather" of the Patient Centered Medical Home has spent four decades focused on population health built on a platform of a healing relationship of trust in primary care. Dr. Grundy currently serves as a director of the ACGME, the body responsible for accrediting graduate medical training programs. He is the founding President of the Patient-Centered Primary Care Collaborative, an organization dedicated to advancing a new primary-care model, called the patient-centered medical home; he was also fundamental in creating it in early 2006.

Dr. Grundy is a retired senior diplomat with the rank of Minister Consular U.S. State Department and was Medical Director for the International SOS and Adventist Health. Among a number of recognitions and awards, Dr. Grundy won the NCQA national quality award for 2012 and the American College of Occupational Environmental Medicine 2013 Sappington Memorial Award. Dr. Grundy is also an adjunct professor at the University of Utah Department of Family and Preventive Medicine and at University of California San Francisco Department of Family and Community Medicine. He attended medical school at the University of California San Francisco and earned his Master's Degree in Public Health at the University of California Berkeley.
