Schryver Medical Lab Now Sending Test Results Into the Community Health Record

Long-term and post-acute care providers will now have access to patient laboratory results from Schryver Medical, helping to improve care transitions.

Long-term and post-acute care providers participating with CORHIO currently access patient laboratory test results through the PatientCare 360® Community Health Record. When a care provider signs in to PatientCare 360, they can see what happened at a recent hospital stay, for example, and be more informed when coordinating care for that patient. Another example is when a home health provider is preparing to visit a new patient, they can review recent test results and other clinical information to learn all they can about the patient. Having a complete picture of the patient’s care helps to smooth the “patient handoff” that can often be stressful and rushed for both the patient and provider.

Test Results Available Through Web Portal

Now that Schryver Medical is sending laboratory test results into the health information exchange (HIE), long-term and post-acute care providers will have access to that information via the secure PatientCare 360  portal. There is no additional action required to access this data, in terms of technical or contractual work with CORHIO. If a test result is available for a particular patient, the information will be automatically available in the portal.

Schryver Medical is a specialized laboratory for the long-term and post-acute care market, including home health, assisted living, hospice, long-term care hospitals, assisted-living centers, and skilled nursing. Schryver offers mobile imaging services, including mobile digital X-rays, EKGs, modified barium swallow studies and ultrasounds to long-term and post-acute care facilities. Other services include phlebotomy, medical equipment rental and waste disposal.
