State Innovation Model Cohort 3 Now Open for Applications

CORHIO is a selected Practice Facilitator as well as a Clinical Health Information Technology Advisor to assist practices with many facets of SIM activities, including the cohort 3 application.
The Colorado State Innovation Model (SIM) has released its request for application (RFA) for the third and final cohort of practices. Eligible practices* are invited to submit an application - due January 10th. CORHIO’s team of Transformation Support Services experts are available to help you complete the application, using their experience with the SIM program.
The SIM initiative helps providers integrate behavioral and physical healthcare and succeed with alternative payment models. If your application is accepted, the work is expected to kick off June 2018 and will last a year. Why should you apply to SIM?
The overall goal of SIM is to help 400 primary care practices and four community mental health centers integrate behavioral and physical health during its four year program. SIM offers a framework of support for practices that are building a comprehensive primary care infrastructure and integrating behavioral health and primary care. The program includes support and resources including hands-on, in-office coaching and technical assistance, achievement-based payments, and small grants up to $40,000 to offset hiring costs, office space changes, training, technology upgrades or patient engagement. The SIM office also works to foster closer relationships with public and private payers, including seven major insurance companies and Health First Colorado (formerly Colorado Medicaid).
*Primary care physicians, pediatrics, geriatrics, internal medicine, and urgent care and OB/GYN practices that provide primary care services
