CORHIO and Centura Health Partner to Improve Care for ACO Patients

To improve outcomes for the patients in their managed care network, Centura’s care coordination team receives real-time utilization updates from out-of-network hospitals. So when patients visit another hospital or emergency facility, Centura’s care coordinators know the details right away and can follow up with the patient.

A recent report, Building Custom Solutions for Real-Time Care Coordination, outlines the successful collaboration between CORHIO and the Colorado Health Neighborhood (CHN), Centura’s physician-led managed care network caring for more than 160,000 patients. The report was published by the Accountable Care Learning Collaborative, a learning network of ACO leaders who are dedicated to helping providers succeed in risk-bearing payment models.

Colorado Health Neighborhood is working to improve care of high-needs patients and has armed its care coordination teams with patient event notification reports from CORHIO. The reports alert CHN’s 15 coordinators when a patient in their network visited another hospital or emergency department.

According to the report, “By equipping care coordinators with actionable notifications, these teams could immediately ensure that the appropriate follow-up care is delivered, mitigating expensive readmissions and improving outcomes. CHN designed its platform to seamlessly commence care management for patients identified in the CORHIO data. The platform identifies the patient’s primary care provider, automatically creates a new encounter and care plan, and alerts the assigned care coordinator to follow up within two business days or upon hospital discharge.”
