Ambulatory Care Summaries Now Available in CORHIO Health Information Exchange Network

CORHIO is now displaying thousands of clinical documents, such as Continuity of Care Documents (CCDs) and OASIS assessments, from a number of physician practices, health clinics, and other ambulatory providers in our secure web portal PatientCare 360.
CORHIO is pleased to announce that clinical documents are now available from the following practices in PatientCare 360. These documents are available on the documents tab as static PDFs within the portal.

- Colorado Care Connections Program - a partnership between CORHIO, Quality Health Network and Colorado’s Department of Health Care Policy and Financing
- Colorado Advanced Interoperability Initiative program – a grant awarded last year from the Office of the National Coordinator for Health Information Technology (ONC) to Colorado Dept. of Health Policy & Financing, working with CORHIO and Quality Health Network
- Community Health Partnership - the Regional Care Collaborative Organization based in Colorado Springs enhancing data exchange in the area
