Hundreds of Providers and Staff at One of the Nation’s Largest FQHCs Now Live on CORHIO

With the assistance of health information exchange grant funding from the state, Peak Vista Community Health Centers are now both accessing data from and contributing data to the CORHIO HIE.

With more than 90,000 patients cared for in 26 outpatient centers, Peak Vista Community Health Centers is one of the nation’s largest Federally Qualified Health Centers. Many of Peak Vista’s healthcare providers and support staff recently gained access to CORHIO’s PatientCare 360, which will help improve clinical decision-making and care coordination for patients in the Pikes Peak and East Central regions.

Provider Buy-In Essential for Success

Peak Vista recognized that for the CORHIO launch to be successful, clinical providers had to be on board. 

“What this is really accomplishing is to assure decisions are made at the most appropriate location, in a timely basis, and by the people most knowledgeable on the options and impact,” says Halie Anthony, MBA, Vice President of Information Technology. “We decided to pursue CORHIO because it was right for patient care and quality. So we enlisted a provider team to champion the project – we had the right people at the table, providers and clinical operations driving those decisions based on what data and information they need in order to support quality care.”

Peak Vista Also Sending Care Summaries Into the HIE

As part of their grant program from the state, Peak Vista is also a data contributor to the health information exchange. They auto-generate care summary documents from their NextGen electronic health record system, which are uploaded to PatientCare 360. These documents will soon be available for other providers throughout Colorado to access. This will be particularly helpful for patients with multiple or complex conditions, who see many providers and specialists who need to be kept informed.
