CORHIO and HealthONE Selected to Speak at National CPC+ Meeting

CORHIO’s Chief Operating Officer Kate Horle has been selected to present alongside Lindsey Puckett, Quality Manager at Hospital Corporation of America.

The CPC+ 2018 National Meeting is the first-time national gathering of all participants and stakeholders in the Comprehensive Primary Care Plus (CPC+) program. According to CMS, the CPC+ program “…is a national advanced primary care medical home model that aims to strengthen primary care through regionally-based multi-payer payment reform and care delivery transformation. CPC+ includes two primary care practice tracks with incrementally advanced care delivery requirements and payment options to meet the diverse needs of primary care practices in the U.S.”

The CORHIO and HealthONE session, “More Than Just Admissions, Discharges, and Transfers: Connecting to Health Information Exchanges (HIEs) to Maximize Coordination,” will take place in Baltimore Maryland on May 8, 2018 at 10:15 am. The session will cover ways participants in the CPC+ program can utilize Health Information Exchange to meet program objectives, particularly care coordination.
