Health professionals can now receive data on patients who need housing, transportation and food assistance
More attention is being placed on social determinants of health (SDOH) -- particularly as payers more actively encourage proper coding for SDOH-related reimbursements. With that, CORHIO is pleased to offer a new Notifications type that delivers data on your patients’ social needs referrals, such as housing, transportation and food assistance.
Research has shown that understanding your patients’ complete picture of what affects their health is vital in successful treatment outcomes. In offering Social Needs Referral Notifications, CORHIO worked with major SDOH vendors such as Unite Us and findhelp (formerly Aunt Bertha) to build direct connections to make their data available.
How it works
Similar to CORHIO’s other Notifications products, users of Social Needs Referral Notifications upload their patient files to CORHIO and we match patients using our Master Patient Index. Then, we search for data in our system and deliver your results as a .csv report via CORHIO’s Secure File Transfer Protocol (SFTP).
With CORHIO Notifications, practices receive real-time or daily alerts when a patient has new lab results, has visited an ER or has been admitted or discharged from a hospital and more. As a result, practices can improve care management, reduce hospital readmissions, enhance disease management programs and inform quality improvement programs.
