Southwest Colorado critical access hospital will benefit from bidirectional exchange project with CORHIO.

CORHIO is pleased to announce that data from Pagosa Springs Medical Center (PSMC) is now available to our network participants in the form of Care Summaries, which are also called Continuity of Care Documents (CCDs). This critical access hospital serves a vital need in a community. Prior to PSMC’s opening in 2008, the nearest hospital was 60 miles away in Durango.

Data from PSMC now available in the CORHIO network includes:

  • Clinical encounters
  • Allergies
  • Problems
  • Procedures
  • Medications
  • Immunizations
  • Radiology results
  • Labs
  • Vitals
  • Social history
  • Assessment and plan
  • Hospital discharge instructions

For PatientCare 360® users interested in seeing Pagosa Springs Medical Center data, here’s how to access the data. The workflow is different from a normal search in PatientCare 360 as you will access PSMC data in a CCD format, not as discrete data in the Community Health Record.

“Pagosa Springs Medical Center (PSMC) has participated as a data sender to CORHIO since 2017. PSMC engaged in a bidirectional CCD exchange project with CORHIO to benefit from the ability to receive and incorporate outside record data directly into the Electronic Medical Record.”Susan Schmidt, Healthcare Project Specialist and EHR Strategist, Pagosa Springs Medical Center
