New for 2021: Community Care Summaries in PatientCare 360®

PatientCare 360 is getting even better! A secure web-based portal that physician practices and health clinics access for real-time patient information, PatientCare 360 saves time and provides accurate clinical data at the point of care. It also helps to better inform clinical decisions and patient communication.

CORHIO has continuously improved the portal over the years, and now we’re proud to announce that Community Care Summaries have been added as a PatientCare 360 feature. Community Care Summaries will give healthcare professionals access to a new stream of useful patient data. The summaries are available to physician practices and other PatientCare 360 users at no additional cost.

What are Community Care Summaries?
In short, Community Care Summaries are individual care summaries from a single sender (commonly known as “encounter summaries” from community providers). PatientCare 360 users can now query these summaries from community providers and retrieve a consolidated 12-month view of patient encounters from those providers participating in the CORHIO HIE.

In addition to summaries previously available from Kaiser Permanente, the U.S. Department of Defense, the Veterans Administration and Quality Health Network (an HIE serving Colorado’s Western Slope), summaries from more than 265 providers are now available in PatientCare 360. Senders include Boulder Medical Center, Rocky Mountain Urgent Care and SCL Medical Group. For a full list of community providers sending care summaries to the CORHIO HIE network, see this list.

How to access Community Care Summaries?
The process of accessing encounter summaries from community providers with PatientCare 360 is easy. You can navigate to them via the Documents tab.
