Need help using this page?

There are three ways to find CORHIO participants on this page:

1. Zoom in on the map by clicking the (+) button on the left side, which will narrow the map down to your desired area. Or you can expand using the (-) button, which will zoom the view back out. Click on the map pin point to find more information on the participant.

2. In the list on the main field of the page, you can sort by column headers: Participant Name, Participant Type, City or Status. Click on the name of the participant to find more information in the map view.

3. Use the Search tool at left to search by one or more of these search parameters: Physician Office, Hospital or Clinic Name, City, Zip Code or Radius (5, 10, 25, 50 or 100 miles)

Please Note: The Export function does not work properly with Internet Explorer. Please use Google Chrome or Firefox.

The PDF in the left column “Data Available in the CORHIO Network” lists all data types available in the CORHIO health information exchange system by sender name. This is the data CORHIO participants have access to in the secure web portal PatientCare 360® and/or through their EHR.
